Assessment & Plan

T2DM — improving. HbA1c goal <7.0 approached.

  1. Continue metformin 1000 mg BID — effective and well-tolerated
  2. Increase rosuvastatin to 20 mg — LDL 89 is above the ADA target of <70 for diabetes with additional risk factors
  3. Continue lisinopril 10 mg — BP near goal (<130/80); recheck at next visit
  4. Lifestyle: maintain walking routine, continue dietary modifications
  5. Retinal eye exam referral and urine albumin-to-creatinine ratio (both due annually per ADA guidelines)
  6. Return in 3 months — repeat HbA1c, lipid panel, BMP

How Notat fills this in

1

You talk with the patient

Notat listens in the background. No dictation commands, no typing during the visit.

2

Facts come first

The FactsContext™ engine extracts the clinical facts, each linked to what was said, so you can check them.

3

The note is written from those facts

Notat fills in this template from the checked facts. You review, edit, and sign.

What this format captures
