This is the structure Notat follows when it writes the note from the visit — you never fill it in by hand.

This is to certify that [Patient's Name if mentioned], has been seen and evaluated by me on [Date]. Please be advised that due to medical reasons, they are recommended to refrain from professional responsibilities for the period of [start date-end date] (if mentioned. 

(Do not specify the medical reason or go into detail).
