This is the structure Notat follows when it writes the note from the visit — you never fill it in by hand.
Visit Summary (Concise & Patient-Friendly) (Maximum 5 sentences.) [Briefly explain the possible diagnosis or diagnoses in simple, patient-friendly language, and explain what it is to the patient. Do not list subjective or objective information unless relevant. Use full sentences only.] (Concise & Patient-Friendly) (Maximum 5 sentences.)
